ABSTRACT

Introducción: El envejecimiento periorbitario se considera un proceso complejo y multifactorial que se relaciona con procesos intrínsecos y extrínsecos tales como los cambios degenerativos en la piel, la aparición de líneas finas, la pérdida de volumen, el descenso de tejidos, la piel flácida y el exceso de depósito de grasa. Objetivo: Evaluar los resultados de la corrección de las ojeras y del surco nasoyugal con Nanofat. Métodos: Se realizó un estudio longitudinal prospectivo en el Servicio de Cirugía Plástica y Caumatología del Hospital Clínico Quirúrgico "Hermanos Ameijeiras" desde agosto de 2019 a mayo de 2021. La muestra la constituyeron 37 pacientes. Los datos se resumieron en frecuencias absolutas y relativas. Resultados: La edad promedio fue 47,2 ± 10,7 años. El 89,2 por ciento de los pacientes eran mujeres. La media del tejido emulsionado infiltrado por cada lado fue de 1,40 ± 0,4 ml, con un volumen mínimo de 1 ml y un máximo de 2 ml. En el 83,8 por ciento de los casos se constató una disminución del surco nasoyugal y en el 16,2 por ciento se evidenció una gran mejoría. En el 100 por ciento de los pacientes hubo avance de la hiperpigmentación y mejoría de la calidad de la piel. Dentro de las complicaciones solo un paciente (2,7 por ciento) presentó nódulos por calcificación en la zona receptora. El resto de los pacientes en estudio no presentó eventos adversos. El grado de satisfacción postratamiento con Nanofat fue de 100 por ciento. Conclusiones: Los resultados de la corrección de las ojeras y del surco nasoyugal con Nanofat son muy buenos(AU)


Introduction: Periorbital aging is considered a complex and multifactorial process that is related to intrinsic and extrinsic processes such as degenerative changes in the skin, appearance of fine lines, volume loss, tissue descent, sagging skin, and excess fat deposition. Objetive: To evaluate the results of correction of dark circles under the eyes and the nasojugal sulcus with Nanofat. Methods: A prospective longitudinal study was performed in the Plastic Surgery and Caumatology Service of the Clinical Surgical Hospital "Hermanos Ameijeiras" from August 2019 to May 2021. The sample consisted of 37 patients. The data were summarized in absolute and relative frequencies. Results: The mean age was 47.2±10.7 years. The 89.2 percent of the patients were women. The mean emulsified tissue infiltrated per side was 1.40±0.4 ml, with a minimum volume of 1 ml and a maximum of 2 ml. In 83.8 percent of the cases there was a decrease in the nasojugal sulcus and in 16.2 percent there was a great improvement. In 100 percent of the patients there was improvement of hyperpigmentation and great improvement of skin quality. Among the complications only one patient (2.7 percent) presented nodules due to calcification in the recipient area. The rest of the patients in the study did not present adverse events. The degree of post-treatment satisfaction with Nanofat was 100 percent. Conclusions: The results of correction of dark circles and nasojugal sulcus with Nanofat are very good(AU)

ABSTRACT

Objetivo: Evaluar el efecto de las células madre derivadas del tejido adiposo en el tratamiento de las arrugas periorbitarias. Métodos: Ensayo comparativo aleatorizado doble ciego en el Servicio de Cirugía Plástica del Hospital Ameijeiras, en el período comprendido entre septiembre de 2019 y julio de 2021. La muestra quedó formada por 70 pacientes que cumplieron los criterios de selección que se distribuyeron en dos grupos de tratamiento. Resultados: Predominaron las mujeres (88,6 por ciento) con promedio de edad de 47 años sin diferencias significativas entre los grupos de tratamiento. El 60,0 por ciento del grupo estudio y el 62,9 por ciento del control presentaron fototipo cutáneo II sin diferencias significativas entre los grupos (p= 0,855). El mayor porcentaje del grado de envejecimiento correspondió al III y el menor al IV (14,3 por ciento contra 20,0 por ciento), sin diferencias significativas entre los grupos (p= 0,487). Antes del tratamiento, hubo mayor frecuencia de arrugas moderadas (estudio: 42,9 por ciento; control: 51,4 por ciento) y severas (estudio: 42,9 por ciento; control: 34,3 por ciento). Posterior a la intervención, se constató una mejoría superior en el grupo estudio con cambios significativos en ambos conjuntos tratados (p< 0,001). Se presentó una complicación del grupo control (2,9 por ciento); 100 por ciento quedó satisfecho y los resultados fueron buenos en el grupo estudio (94,3 por ciento) y regulares en igual proporción del control con diferencias significativas (p< 0,001). Conclusiones: La lipotransferencia asistida con células madre ofrece resultados superiores a la convencional en el rejuvenecimiento periorbitario(AU)


Objective: To evaluate the effect of adipose tissue-derived stem cells in the treatment of periorbital wrinkles. Methods: Randomized double-blind comparative trial in the Plastic Surgery Service of the Clinical Surgical Hospital "Hermanos Ameijeiras" in the period since September/2019 until July/2021. The sample consisted of 70 patients who met the selection criteria and were distributed into two treatment groups. Results: Women predominated (88.6 percent) with an average age of 47 years with no significant differences between the treatment groups. The 60.0 percent of the study group and 62.9 percent of the control group presented skin phototype II with no significant differences between the groups (p= 0.855). The highest percentage of aging corresponded to III and the lowest to IV (14.3 percent vs. 20.0 percent), with no significant differences between the groups (p= 0.487). Before treatment, there was a higher frequency of moderate (study: 42.9 percent; control: 51.4 percent) and severe wrinkles (study: 42.9 percent; control: 34.3 percent). After the intervention, there was a superior improvement in the study group with significant changes in both treated groups (p< 0.001). There was one complication in the control group (2.9 percent). 100 percent were satisfied and the results were good in the study group (94.3 percent) and regular in equal proportion of the control with significant differences (p< 0.001). Conclusions: Stem cell-assisted lipotransfer offers superior results to conventional lipotransfer in periorbital rejuvenation(AU)

ABSTRACT

Introducción: En el tejido adiposo se han identificado células madre mesenquimales con capacidad autorrenovadora y multipotencial. Mediante digestión enzimática y centrifugado del lipoaspirado se libera una población heterogénea de células denominada fracción vascular estromal, con innumerables potencialidades terapéuticas en el campo de la medicina regenerativa. Objetivo: Actualizar el alcance de las células madre derivadas de tejido adiposo en la terapia regenerativa. Método: Se revisaron 38 artículos entre los años 2000 y 2019 en las bases de datos Scielo, ScienceDirect, Medline y Pubmed. Desarrollo: Las células de la fracción vascular estromal se caracterizan por su capacidad de generar tejido adiposo y vasos sanguíneos, y por la producción de factores de crecimiento que ayudan en la supervivencia de los adipocitos y la formación de una red vascular. El principal mecanismo de acción de las células madre derivadas de tejido adiposo parece deberse a su acción paracrina y a la sinergia con células endoteliales. En el campo de la medicina regenerativa se han utilizado en el tratamiento de cicatrices patológicas y de fibrosis deformantes con impotencia funcional, en las reconstrucciones de secuelas por cáncer y en el cierre precoz de zonas cruentas. Conclusiones: La lipotransferencia es un procedimiento con un mínimo de complicaciones que constituye una de las opciones terapéuticas más empleadas para corregir defectos en los tejidos, debido a que no solo es un medio de relleno, sino que también permite la regeneración y restauración tisular. La presencia de células madre en el tejido adiposo, unido a su accesibilidad, disponibilidad e histocompatibilidad, ha motivado su aplicación cada vez más expandida en la medicina estética, reconstructiva y regenerativa(AU)


Introduction: Mesenchymal stem cells with self-renewing and multipotential capacity have been identified in adipose tissue. By means of enzymatic digestion and centrifugation of the lipoaspirate a heterogeneous population of cells called vascular stromal fraction is released. It has innumerable therapeutic potentialities in the field of regenerative medicine. Objective: To update the scope of stem cells derived from adipose tissue in regenerative therapy. Method: 38 articles published between 2000 and 2019 in the Scielo, ScienceDirect, Medline and Pubmed databases were reviewed. Development: The cells of the vascular stromal fraction are characterized by generating adipose tissue and blood vessels and by the production of growth factors that help in the survival of adipocytes and the formation of a vascular network. The main mechanism of action of stem cells derived from adipose tissue appears to be due to their paracrine action and synergy with endothelial cells. Stem cells derived from adipose tissue have been used in regenerative medicine for the treatment of pathological scars and deforming fibrosis with functional impotence, in the reconstruction of cancer sequelae and in the early closure of bloody areas. Conclusions: Lipotransfer is a procedure with a minimum of complications that constitutes one of the most widely used therapeutic options to correct tissue defects, since it is not only a filling medium, but also allows tissue regeneration and restoration. The presence of stem cells in adipose tissue, together with their accessibility, availability and histocompatibility, has motivated their increasingly widespread application in aesthetic, reconstructive and regenerative medicine(AU)

RESUMEN Se presentó el caso de un paciente masculino de 58 años, con antecedentes de salud aparente, que refiere haber recibido tratamiento quirúrgico por fractura compleja de tercio medio facial (fracturas tipo Lefort II y malar III derecha), el mismo refiere inconformidad estética por la deformidad facial postraumática que presenta. Se decidió su ingreso en el Servicio de Cirugía Maxilofacial del Hospital Universitario "General Calixto García" para su tratamiento. Se diagnosticó deformidad postraumática secundaria a una fractura facial compleja y su consiguiente daño estético. Los resultados alcanzados con la utilización de la lipotransferencia de tejido graso abdominal para el tratamiento de la deformidad facial fueron satisfactorios con permanencia y estabilidad en un año, a la vez que cumplió con las expectativas del paciente. La lipotransferencia resulta una alternativa viable en el tratamiento de las deformidades faciales adquiridas.


ABSTRACT The case of a 58-year-old male patient was presented, with a history of apparent health, who reported having received surgical treatment for a complex fracture of the middle facial third (fractures Lefort II and right malar III), the same refers to aesthetic disagreement for the deformity posttraumatic facial presenting. It was decided to enter the Maxillofacial Surgery Service of the University Hospital "General Calixto García" for treatment. Posttraumatic deformity was diagnosed secondary to a complex facial fracture and its consequent aesthetic damage. The results achieved with the use of abdominal fatty tissue lipotransfer for the treatment of facial deformity were satisfactory with permanence and stability in one year, while meeting the patient's expectations. Lipotransfer is a viable alternative in the treatment of acquired facial deformities.


RESUMO Foi apresentado o caso de um paciente do sexo masculino, 58 anos, com histórico aparente de saúde, que relatou ter recebido tratamento cirúrgico para uma fratura complexa do terço facial médio (fraturas de Lefort II e malar III direita), o mesmo se refere a discordância estética quanto à deformidade apresentação facial pós-traumática. Foi decidido entrar no Serviço de Cirurgia Maxilofacial do Hospital Universitário "General Calixto García" para tratamento. A deformidade pós-traumática foi diagnosticada secundária a uma fratura facial complexa e seu conseqüente dano estético. Os resultados obtidos com o uso da lipotransferência de tecido adiposo abdominal para o tratamento da deformidade facial foram satisfatórios com permanência e estabilidade em um ano, atendendo às expectativas do paciente. A lipotransferência é uma alternativa viável no tratamento de deformidades faciais adquiridas.

ABSTRACT

Objective To evaluate the therapeutic effect of autologous fat grafting in secondary cleft lip.Methods Grafting of autologous fat granules was applied to correct lip under-capacity.The technique was used in 40 patients with secondary cleft lip from June 2011 to December 2015.The ratios between the point of upper lip peak to vermilion height and to white lip height on the affected side were measured by Photoshop CS6.Results Satisfactory results were achieved with all 40 patients;the lip contour and morphology improved.The preoperative ratio was 0.417±0.190 and the postoperative ratio was 0.499±0.197, which increased average 26.02%, with statistically significant (P<0.05).Conclusions Autologous fat grafting to correct lip under-capacity with secondary cleft lip is a safe, effective and micro-invasive method which can be sustained over a lengthy period.

ABSTRACT

Progressive facial hemiatrophy, also known as Parry-Romberg syndrome, is a progressive and self-limited deformation of the subcutaneous tissue volume on one side of the face that creates craniofacial asymmetry. We present the case of a patient with a five-year history of progressive right facial hemiatrophy, who underwent facial volumetric restoration using cell-assisted lipotransfer (CAL), which consists of an autologous fat graft enriched with adipose-derived stem cells (ASCs) extracted from the same patient. ASCs have the capacity to differentiate into adipocytes. They also promote angiogenesis, release angiogenic growth factors, and some can survive as stem cells. The use of autologous fat as a filler in soft tissue atrophy has been satisfactory in patients with mild and moderate Parry-Romberg syndrome. Currently, CAL has showed promising results in the long term by decreasing the rate of fat reabsorption. The permanence and stability of the graft in all the injected areas has showed that autologous fat grafts enriched with stem cells could be a promising technique for the correction of defects caused by this syndrome.
